Switches port-proxy from requiring NodePort services to headless
ClusterIP services. (i.e. clusterIP == "None"). Nginx can then use DNS
for round-robin load balancing.

Adds a filter for turning (IPAddress, port) tuples into a format
suitable for nginx.
